Copy of a valid ID of Principal
Current Account at MCB
Completed application form and questionnaire
Two references
Business Plan (if applicable)
Company Articles of Incorporation (if applicable)
Premises rental agreement (if applicable)
Site Inspection
Signed Electronic Merchant Agreement & Maestro Merchant Agreement
Signed Amex Affiliation form (If desired)
